Ultrasound Guided Medial Branch Block for the Treatment of Chronic Cervical Facet Joint Pain

Abstract
The cervical facet joints are a common source of chronic neck pain. Nerve blocks of the nerves supplying the joints are validated tools in the diagnosis and therapy for facetjoint pain. These interventions are typically performed with fluoroscopic guidance. However, the target, that is, the nerve, is radiolucent and cannot be identified with fluoroscopy.Ultrasound allows visualization of the cervical medial branches in most patients and thus the medication can be injected to the target nerves as close as possible. This review illustrates that ultrasound maybe equivalent or superior to fluoroscopy in terms of effectiveness, safety and efficiency (decreased performance time and fewer needle passes) for treatment of cervical facet joint pain. However ultrasound has limitations. Although we have previously established that ultrasound guidance can help prevent intravascular injection, it still remains unclear whether it allows for complete avoidance of intravascular injection. Two methodsare recommended to overcome these limitations. The first is to inject non-particle steroids and the second is to use the lidocaine test before injections.
